What is this mesh?



What do you see in this angiogram? Looks like mesh with roughly an H shape. As most of you already know, this is an ASD Closure Device just after successful implantation. The 2 years old child had an ASD secundum as you can see in the following Apical 4 Chamber view,

                          

The ASD is quite prominent, and meausered 22mm in one plane. An echo was performed right after the procedure which showed the Device holding in its place with no residual shunting. The following is the Subcostal view that shows the device in place:

This is a modified view just to highlight the Device and only LA is clearly visible in the center of the image and the bright part is the device. 

                                     

 The procedure is simple, first insert a catheter through the right femoral vein into the RA and navigate through the ASD into LA and through LA into one of the Pulmonary veins (usually RUPV). Inject some die to confirm position and then place a wire. Usually the diagnostic catheter is withdrawn and another catheter or long sheath is inserted over the wire and once in position the guide wire is removed. Through this the ASD device is inserted. ASD device is mesh and is collapsed and inserted into the catheter. It opens up like an umbrella when it comes out from the other end of the catheter.
