Subclavian Vein Stenosis after insertion of a Permanent Pacemaker

This is an Angiogram of the Left Subclavian vein showing stenosis inside it. 2 Pacemaker leads are visible inside the vein and the device can also be partially seen at the edge of the image.
This patient had a Permanent Pacemaker implanted 3 years back and presented with discharge from the insertion site, ex-plantation was planned and during the procedure he was noted to have developed stenosis of the left subclavian vein. The wires were adhering to the walls and were removed under surgical cover.

ReplyDeleteUsually it is inserted through the subclavian vein (behind the collar bone) as you mentioned. Alternative route is through the lower limb veins, keeping the pacemaker in the lower part of the abdomen. Still another route is with a surgery to open the covering of the heart and keeping the electrode on the surface of the heart and the pacemaker in the upper part of the abdomen (tummy).
