RCA dissection extending into Root of Aorta during Diagnostic Angiography
This patient was undergoing diagnostic coronary angiography, when he developed a dissection in his RCA. This dissection quickly ascended into the Aortic wall. The first few frames of the above video show filling of the RCA, which already has critical disease with lots of calcium. Then a dissection appears in the RCA, after which dye can be seen ballooning up into the wall of Aorta causing an Iotarogenic Dissection.
The left system was also diseased and the patient already had good collaterals to the RCA from left system. The patient remained vitally stable and was shifted to ICU, bedside echo showed no significant findings. The patient was treated conservatively, owing to the stable condition of the patient. He remained in ICU for 2 days and in step down for a further 2 days after which he was discharged. The patient was stable at followup after 01 month.
This video highlights the rare but gruesome complications that may happen during diagnostic angiographies.
